二、报名供应商资格要求
1、在中国境内注册，具有独立法人资格，持有合法的营业执照。
2、具备财政部门颁发的会计师事务所执业证书。
3、具有良好的职业道德记录和信誉，近3年内没有受相关国家行业管理机构通报批评、停业整顿的记录；近3年内经营活动中没有重大违法记录。
4、单位负责人为同一人或者存在直接控股、管理关系的不同供应商，不得同时参与本采购项目。
5、本项目不接受联合体投标，不允许转标和分标。
三、报名时提交的资料清单、要求及投递方式
1、资质文件
（1）企业法人营业执照或合伙企业营业执照；
（2）会计师事务所执业证书；
（3）法人代表证明书、法人代表第二代居民身份证复印件；
（4）法人代表授权书；
（5）授权代理人第二代居民身份证复印件及近三个月社保缴费证明；
（6）服务业绩记录（2020年6月至本公告之日止近三年，合同或中标通知书复印件）；
（7）近3年无通报批评、停业整顿、重大违法记录承诺函（格式自定）。
